GithubFlavoredMarkdownTable

GithubFlavoredMarkdownTable was initially implemented bcho_. It produces a `GitHub Flavored Markdown`_ formatted table.

Because there are no outer table borders:

* Table titles are ignored.
* Border display toggles are also ignored.
