Open the bibliography database table <emph>biblio</emph> in the data source view. While pressing Shift+<switchinline select="sys"><caseinline select="MAC">Command</caseinline><defaultinline>Ctrl</defaultinline></switchinline>, drag a few column headers into the document so that the form fields are created.</paragraph>
